I suggest that the speaker has uttered a colorful humorous metaphor that refers to a common pre/contraceptive pill situation and context which was:

When a female did not want to have sexual relations, she responded: "I have a headache."

The aspirin between the knees! Get it?

Not many Americans have.

Which I suggest is a sad commentary on the extent of our knowledge of metaphors, of cultural history, and on the state of our sense of humor.
