The UK General Medical Council states that it is the duty of a doctor to report a colleague who may be underperforming, and not actually caused harm, and if one does not do so then one is culpable too. What is the ethical basis for this view that I am my brothers keeper? How can I be responsible for the consequences of anothers actions?

Hmm, just guessing...

Guessing that they are asking you the well performing doctor to be responsible for your own job. Which the General Medical Council seems to be defining as bringing accurate health care information to the public.

If you the well performing doctor decline to provide me the public/patient with important information that may impact my health, isn't your credibility, and thus ability to serve the public impacted?
